Telegraph: Americans begin new offensive in Fallujah
The US military has launched an operation against insurgents around Fallujah only six months after its officials proclaimed the city “the safest place in Iraq”.

The 2 statements are not necessarily incompatible.
“Safest place in Iraq” is a relative term of virtually no value.
Much anecdotal evidence that hi level security / mercenary companies are having problems recruting / keeping staff.
